Dr. Jacob Mosko

PT, DPT
Physical Therapist

I grew up in Los Gatos, California as the middle child of two brothers. Baseball was my passion but I tore a ligament in my elbow at the age of 13. After nine months of rehab I fell in love with the prospect of being a physical therapist.

I attended the University Of Oregon (UO) and received my BS in Human Physiology. While at UO, I competed for two years on the club rowing team and one year on the ski and snowboarding team.

After undergrad, I attended Columbia university for PT school. I participated in research with the baseball team and continued my passion for teaching by volunteering as a teaching assistant in the anatomy lab and with the biomechanics courses. I continued to grow my passion for weight training, leading me to Forefront, which I feel is the perfect intersection of my passion and career.

While at Columbia, I completed 4 clinical rotations to build my clinical experience while working with complex patient presentations. Two of my rotations focused on outpatient orthopedics and sports. My second rotation at New York Presbyterian gave me exposure in working with vestibular and neuro-based patients. My only inpatient experience was at OHSU in Portland, Oregon working in the acute care trauma wing. Each of my clinical instructors made a meaningful contribution to my career by encouraging me to never stop learning.

My core values are equity, access to care, respect for oneself and others, honesty and accountability.

“Motion is Lotion” – Dr. Rami Said (Kinesiology and Biomechanics professor)

I specialize in weight lifting, biomechanical analysis, running analysis, sports focused rehab and overhead athlete arm care (baseball, tennis, volleyball, etc). Additionally, I enjoy treating generalized back pain, chronic pain, acute overuse injuries, post operative care (all types of surgical recoveries) and rehab focused on functional tasks (gardening, doing laundry, etc). I will gladly take any patient that is willing to put in the required effort to get better.

I put an emphasis on improving mindset and outlook within the context of rehab which leads to better outcomes with my patients. Something many patients deal with is the fear of movement which leads to higher levels of pain.  Helping patients better understand their pain and fear leads to an improved mindset and a faster recovery.

Working at Forefront Physical Therapy gives me the freedom to utilize a large range of equipment in a high energy work environment.  Forefront Physical Therapy also provides incredible mentorship and growth opportunities and lets me live and work in an excellent location.

When i’m not working, I enjoy hiking, lifting, camping, golfing, ping pong/pickleball, playing with my dogs and gaming with my friends.
